Indication: Treatment of established DVT with or without pulmonary embolism.
Dosage: General surgery with moderate risk of venous thromboembolism: 2500 IU SC, 2 hrs before or 6 hrs after surgery; then 2500 IU SC every 24 hrs on subsequent days. Orthopaedic surgery with a high risk of venous thromboembolism: 3500 IU SC, 2 hrs before or 6 hrs after surgery; then 3500 IU SC every 24 hrs on subsequent days. Maintain prophylactic treatment for at least 7-10 days after the surgical procedure and until the risk of thromboembolic disease has decreased. Non-surgical patients with moderate to high risk of venous thromboembolism: 2500 IU/day or 3500 IU/day SC. Recurrent venous thromboembolism in patients with DVT and transient risk: 3500 IU/day (3 mos) in patients who have received appropriate initial anticoagulant treatment for DVT with or without pulmonary embolism. Extracorporeal circuit during hemodialysis – Patients <60 kg: 2500 IU; >60 kg: 3500 IU. Single dose in the arterial line at the beginning of the dialysis session. Treatment of DVT – Patients >100 kg: 115 IU/kg; >70 kg: 10000 IU; 50-70 kg: 7500 IU; <50 kg: 5000 IU. Administered once daily for 5-9 days.
